Identify the view, anatomy, type of map, and interpretation of the following 3D image.
Answer
This is an LAT map of the RA from a Rhythmia system. The image is viewed from the AP slightly caudal perspective. The cavotricuspid isthmus (CTI) is displayed, but this is not a CTI dependent flutter. This is a focal atrial tachycardia with the earliest activation site at approximately 6 o’clock on the tricuspid valve. Ablation at this early activation site terminated the rhythm. We can tell this is focal as the activation spread outward in all directions from the earliest site.
Propagation Map:
In this same view, you may visualize the wave of depolarization traveling out from the earliest activation site in all directions. With this system, the earliest activation is red, followed by orange, yellow, green, blue, and purple. This is the appearance of a focal tachycardia. The ablation site observed terminated the rhythm. Atrial tachycardia is mapped and ideally ablated during tachycardia.
